PROFESSIONAL DISCLOSURE STATEMENT

Philosophy and Approach: I work collaboratively with clients to understand their current experience and identify workable steps to precipitate change in order to better serve their life goals. I encourage active engagement in treatment to better support greater and long-term desired change. The techniques I use are supported by current evidence; this approach to therapy is often called “Evidence-Based Practice.” My general counseling approach draws from Acceptance and Commitment Therapy (ACT), Motivational Interviewing (MI), and Community Reinforcement and Family Training (CRAFT) techniques.

Formal Education and Training: I hold a masters degree in counseling psychology from the Pacific University School of Professional Psychology. My coursework and clinical training have focused on using behavioral approaches to provide counseling to adults with mental health and addictions concerns in both individual and group settings. I am a Certified CRAFT Clinician through Dr. Robert J. Meyers and associates.

Licensing/Credentialing and Ethics: As a Licensed Professional Counselor (LPC) of the Oregon Board of Licensed Professional Counselors and Therapists, I abide by its Code of Ethics and that of the American Counseling Association. To maintain my license I am required to participate in continuing education, taking classes dealing with subjects relevant to this profession. Additionally, as a Certified Alcohol Drug Counselor I (CADC I) through the Addiction Counselor Certification Board of Oregon, I abide by its ethical standards and complete continuing education as required to maintain my certification. I regularly participate in supervision and/or consultation groups to continue to enrich my practice and perspective.

Fees and Payment Policies: Initial consultations (30 minutes) are complimentary. Individual counseling sessions (50 minutes) are provided at a self-selected range of $100-$160 per session. Additionally, I reserve a limited number of lower-fee openings; if your financial situation is such that you cannot afford my standard-fee range, please inquire about current availability. Multi-participant sessions and group therapy fees vary depending on the number of participants and length of sessions.

During the initial consultation, prior to the start of their first counseling session, all clients will be made aware of the financial obligations pertaining to their therapy and a Fee Agreement will be signed at that time.

Payment is due at the beginning of each session. Please cancel appointments at least 24 hours in advance to avoid being charged for the session. I do not accept insurance at this time.

Patient Rights: As the client of an Oregon Licensed Professional Counselor, you have the following rights:

To expect that a licensee has met the minimal qualifications of training and experience required by state law ・To examine public records maintained by the Oregon Board of Professional Counselors and Therapists and to have the Board confirm credentials of a licensee・To obtain a copy of the Code of Ethics for Professional Counselors・To report complaints to the Oregon Board of Professional Counselors and Therapists・To be informed of the cost of professional services before receiving the services・To be assured of privacy and confidentiality while receiving services as defined by rule and law, excluding the following exceptions: reporting suspected child abuse, elder abuse, abuse of disabled adults, or animal abuse; reporting imminent danger to client or others; reporting information required in court proceedings or by client’s insurance company, or other relevant agencies; providing information concerning licensee case consultation or supervision; defending claims brought by client against licensee ・To be free from discrimination on the basis of race, religion, gender identification or expression, sexual orientation, or other unlawful category while receiving services.
